Patient Assistance & Community Commitment

Community Commitment:

Restoring Health Mobile Medical LLC (RHMM) is a for-profit mobile primary care practice dedicated to improving access to healthcare for adults in Duval, Nassau and surrounding counties. 

We understand that financial challenges and access barriers can make it difficult for some individuals to receive timely medical care.   While RHMM is not a nonprofit organization, we are committed to serving our community in a responsible and ethical manner.

Patient Assistant Program:

RHMM offers limited patient assistance options for eligible individuals experiencing temporary financial hardship.

Assistance may include:

·  Reduced fee visits 

   ·  Limited pro bono primary care visits

Availability is based on clinical capacity and practice resources.

Participation in the Patient Assistant Program is:

·  Voluntary

·  Not guaranteed

·  Not required to receive care

All patients receive the same quality and standard of medical care regardless of assistance eligibility.

Important Information:

·  RHMM does not request or accept donations from patients in exchange for care

·  Medical care is never contingent on financial assistant participation

·  Assistance is offered on a limited time basis for approved individuals
